Access to Secure Health Data Patient Portal
Following your visit, you should have received an email from our office. Inside this email is a link to the Secure Health Data Portal. You can use this portal to access your Electronic Medical Records from our office.
To access the portal and your records:
- Click on the link in the e-mail.
- Once page opens, click “Secure Login”.
- Enter your username (this will be the email address that you gave our office) and click “login in”.
- You will then immediately receive another email with Validation Code. Enter this when prompted to.
- The system will then ask you to create a new password and assign your secret question. (You will need both of these each time you log on to the system.)
- Next, the system will ask you to log in again with the new password.
You have now set up your account.
In the future, all you will have to do is enter your email address, password, and the answer to the secret question when logging in.
